<sec><title>연구배경</title>
<p>본 연구는 고령화연구패널조사(Korean Longitudinal Study of Ageing)를 활용하여 지역사회 거주 노인의 동거형태에 따라 8년간의 인지기능 변화 양상을 파악하고, 독거노인과 동거가족이 있는 노인의 인지기능 변화 유형별 영향요인을 파악하고자 시도되었다.</p></sec>
<sec><title>방법</title>
<p>본 연구의 대상자는 지역사회 거주 65세 이상 노인으로 8년간(2012-2020) 3회 이상 인지기능 측정에 참여한 최종 3,049명의 자료를 분석하였다. 노인의 동거형태에 따른 인지기능 변화의 잠재계층 수의 결정은 잠재계층성장모형(latent class growth modeling)으로 분석하였다. 다항 로지스틱 회귀분석을 활용하여 인지기능 변화궤적의 잠재계층 유형별 영향요인을 확인하였다.</p></sec>
<sec><title>결과</title>
<p>본 연구 결과 독거노인의 경우 &#x02018;높음-유지형(35.6%)&#x02019;, &#x02018;높음-저하형(32.5%)&#x02019;, &#x02018;보통-급감형(6.2%)&#x02019;, &#x02018;낮음-유지형(18.9%)&#x02019;, &#x02018;낮음-저하형(6.8%)&#x02019;의 5개 유형의 인지기능 변화 양상을 보였다. 동거가족이 있는 노인의 경우 &#x02018;높음-유지형(46.4%)&#x02019;, &#x02018;높음-저하형(32.0%)&#x02019;, &#x02018;낮음-유지형(15.9%)&#x02019;, &#x02018;낮음-저하형(5.7%)&#x02019;의 4개 유형의 인지기능 변화 양상을 보였다. 독거노인과 동거가족이 있는 노인 모두에서 연령, 도구적 일상생활 수행 능력, 우울이 인지기능 변화 양상의 공통된 영향요인이었다. 단, 독거노인의 경우 가구 월소득이, 동거가족의 경우 성별, 교육 수준, 체질량지수, 규칙적 운동, 일상생활 수행 능력, 직업, 거주지역이 인지기능 변화 양상을 결정짓는 영향요인으로 나타났다.</p></sec>
<sec><title>결론</title>
<p>본 연구를 통해 동거형태별 노인의 인지기능 변화 양상은 다를 수 있음을 확인하였다. 따라서 본 연구 결과를 바탕으로 노인의 노년기 동거형태별 인지기능 유지 또는 인지기능 손상 예방을 위한 맞춤형 중재개발을 위한 후속 연구와 보건정책마련이 요구된다.</p></sec>

<sec><title>Background</title>
<p> The purpose of this study was to describe the distinct cognitive trajectories for 8 years according to living arrangement among community-dwelling older adults using the Korean longitudinal study of ageing.</p></sec>
<sec><title>Methods</title>
<p> We analyzed data from a final sample of 3,049 community-dwelling older adults aged 65 years or older who participated in at least three cognitive function measures in an 8-year period (2012-2020). Cognitive function was assessed by Korean mini mental state examination. Latent class growth model analysis was applied to determine the number and shape of trajectories of older adults according to living arrangement. Multinomial logistic regression analysis was used to determine risk factors for cognitive function trajectories.</p></sec>
<sec><title>Results</title>
<p> We identified five trajectory patterns in cognitive function among older adults living alone; &#x02018;high-stable (35.6%)&#x02019;, &#x02018;high-declining (32.5%)&#x02019;, &#x02018;moderate-steep declining (6.2%)&#x02019;, &#x02018;low-stable (18.9%)&#x02019;, and &#x02018;low declining (6.8%)&#x02019;. In older adults living with family, there were four trajectory patterns; &#x02018;high-stable (46.4%)&#x02019;, &#x02018;high-declining (32.0%)&#x02019;, &#x02018;low-stable (15.9%)&#x02019;, and &#x02018;low-declining (5.7%)&#x02019;. Age, instrumental activities of daily living, and depression were common factors of cognitive changes in both older adults living alone and those living with family members. However, monthly incomes was only associated with the risk of cognitive function in older adults living alone.</p></sec>
<sec><title>Conclusions</title>
<p> We found that cognitive function trajectories among older adults may differ depending on the type of living arrangement. Therefore, further studies and health policies are required for avoiding cognitive impairment by living arrangement among older adults based on the results of this study.</p></sec>

<title>서 론</title>
<sec>
<title>1. 연구 필요성</title>
<p>전 세계적으로 생활수준 향상 및 의학기술의 발전으로 평균 수명이 연장됨에 따라 노인 인구가 급속도로 증가하고 있는 가운데 우리나라도 2022년 현재 65세 이상의 노인 인구 비율이 전체 인구의 17.5%로, 2025년에는 20%를 초과하여 초고령사회로 진입할 것으로 전망된다&#x0005b;<xref ref-type="bibr" rid="b1-kjhp-2023-23-3-105">1</xref>&#x0005d;. 생물학적 관점에  인간의 노화는 나이가 들어감에 따라 신체적, 정신적, 인지적, 사회적 기능이 저하되는 특징을 보이는데&#x0005b;<xref ref-type="bibr" rid="b2-kjhp-2023-23-3-105">2</xref>&#x0005d;, 이 중에서 도 인지기능 저하는 노인의 삶의 질을 가장 위협하는 요인으로 손꼽히고 있다&#x0005b;<xref ref-type="bibr" rid="b2-kjhp-2023-23-3-105">2</xref>,<xref ref-type="bibr" rid="b3-kjhp-2023-23-3-105">3</xref>&#x0005d;.</p>
<p>고령일수록 기억력을 포함한 언어능력, 지남력, 판단력 등 다양한 인지기능 영역에서의 기능 저하는 물론.&#x0005b;<xref ref-type="bibr" rid="b4-kjhp-2023-23-3-105">4</xref>&#x0005d;, 경도인지 장애 및 치매 이환에 더욱 취약하게 된다&#x0005b;<xref ref-type="bibr" rid="b3-kjhp-2023-23-3-105">3</xref>,<xref ref-type="bibr" rid="b4-kjhp-2023-23-3-105">4</xref>&#x0005d;. 실제로 2022년 기준 국내 65세 이상 노인의 경우 경도인지 장애와 치매 유병률이 각각 15.0%와 10.3%로 나타났고, 특히 85세 이상 노인의 경우 경도인지 장애와 치매 유병률은 각각 22.3%와 38.7%로 더욱 높아지는 것으로 조사되었다&#x0005b;<xref ref-type="bibr" rid="b5-kjhp-2023-23-3-105">5</xref>&#x0005d;. 노인은 젊은 연령층에 비해 한 가지 이상 만성 질환을 앓고 있는 경우가 많은데&#x0005b;<xref ref-type="bibr" rid="b4-kjhp-2023-23-3-105">4</xref>,<xref ref-type="bibr" rid="b6-kjhp-2023-23-3-105">6</xref>&#x0005d;, 인지기능 저하 또는 인지기능이 손상된 만성 질환 노인의 경우 약물 복용 불이행을 포함한 생활습관 수정 등에 있어 효과적 건강관리가 어려워 질병을 더욱 악화시키거나, 이차적인 건강 문제를 초래할 가능성이 높아진다&#x0005b;<xref ref-type="bibr" rid="b7-kjhp-2023-23-3-105">7</xref>&#x0005d;. 또한 인지기능 저하와 관련된 의사소통 장애로 다양한 사회활동이 제한되고 일상생활 수행의 어려움으로 인해 타인의 도움이 필요하게 되면 결국 요양기관 입소로 이어지게 된다&#x0005b;<xref ref-type="bibr" rid="b3-kjhp-2023-23-3-105">3</xref>,<xref ref-type="bibr" rid="b8-kjhp-2023-23-3-105">8</xref>,<xref ref-type="bibr" rid="b9-kjhp-2023-23-3-105">9</xref>&#x0005d;. 따라서 노인의 인지기능 저하를 지연시키거나 예방하는 것은 성공적인 노화의 필수적인 요소임을 고려할 때이며&#x0005b;<xref ref-type="bibr" rid="b2-kjhp-2023-23-3-105">2</xref>&#x0005d;. 지역사회 거주 노인의 인지기능 저하에 영향을 미치는 요인을 파악하는 것은 향후 치매 예방 관련 보건정책 수립 및 인지기능 향상 프로그램 개발을 위해 중요하겠다.</p>
<p>지금까지 수행된 노인 인지기능 저하 또는 손상 관련 영향요인에 관한 국내외 선행 연구를 살펴보면, 노인의 인지 기능은 연령&#x0005b;<xref ref-type="bibr" rid="b4-kjhp-2023-23-3-105">4</xref>,<xref ref-type="bibr" rid="b6-kjhp-2023-23-3-105">6</xref>&#x0005d;, 성별&#x0005b;<xref ref-type="bibr" rid="b6-kjhp-2023-23-3-105">6</xref>,<xref ref-type="bibr" rid="b8-kjhp-2023-23-3-105">8</xref>&#x0005d;, 교육 수준&#x0005b;<xref ref-type="bibr" rid="b4-kjhp-2023-23-3-105">4</xref>,<xref ref-type="bibr" rid="b6-kjhp-2023-23-3-105">6</xref>,<xref ref-type="bibr" rid="b9-kjhp-2023-23-3-105">9</xref>&#x0005d;, 소득 수준&#x0005b;<xref ref-type="bibr" rid="b6-kjhp-2023-23-3-105">6</xref>,<xref ref-type="bibr" rid="b9-kjhp-2023-23-3-105">9</xref>&#x0005d; 등의 인구사회학적 요인과 흡연&#x0005b;<xref ref-type="bibr" rid="b10-kjhp-2023-23-3-105">10</xref>&#x0005d;, 음주&#x0005b;<xref ref-type="bibr" rid="b8-kjhp-2023-23-3-105">8</xref>,<xref ref-type="bibr" rid="b11-kjhp-2023-23-3-105">11</xref>&#x0005d;, 체질량지수&#x0005b;<xref ref-type="bibr" rid="b7-kjhp-2023-23-3-105">7</xref>,<xref ref-type="bibr" rid="b11-kjhp-2023-23-3-105">11</xref>&#x0005d;, 신체운동&#x0005b;<xref ref-type="bibr" rid="b8-kjhp-2023-23-3-105">8</xref>,<xref ref-type="bibr" rid="b10-kjhp-2023-23-3-105">10</xref>&#x0005d;, 만성 질환의 수&#x0005b;<xref ref-type="bibr" rid="b8-kjhp-2023-23-3-105">8</xref>,<xref ref-type="bibr" rid="b11-kjhp-2023-23-3-105">11</xref>&#x0005d;, 일상생활 수행 능력&#x0005b;<xref ref-type="bibr" rid="b9-kjhp-2023-23-3-105">9</xref>,<xref ref-type="bibr" rid="b11-kjhp-2023-23-3-105">11</xref>&#x0005d; 등의 건강 관련 요인 및 우울&#x0005b;<xref ref-type="bibr" rid="b8-kjhp-2023-23-3-105">8</xref>,<xref ref-type="bibr" rid="b11-kjhp-2023-23-3-105">11</xref>&#x0005d;과 같은 심리적 요인이 인지기능에 영향을 미치는 것으로 나타났다. 최근에는 동거형태&#x0005b;<xref ref-type="bibr" rid="b12-kjhp-2023-23-3-105">12</xref>&#x0005d; 또한 인지기능과 연관성이 있는 것으로 보고되고 있으나, 이와 관련된 연구는 주로 국외를 중심으로 이루어지고 있고, 국내 노인 대상 연구는 부족한 실정이다&#x0005b;<xref ref-type="bibr" rid="b13-kjhp-2023-23-3-105">13</xref>-<xref ref-type="bibr" rid="b15-kjhp-2023-23-3-105">15</xref>&#x0005d;. 노인에서의 동거 형태는 배우자와의 사별, 기대 수명의 증가, 노후 부양에 대한 의식 변화 등에 의해 비자발적으로 형성되는 특성을 가지고 있다&#x0005b;<xref ref-type="bibr" rid="b14-kjhp-2023-23-3-105">14</xref>-<xref ref-type="bibr" rid="b16-kjhp-2023-23-3-105">16</xref>&#x0005d;. 노인의 동거형태는 배우자와 손자녀를 포함한 가구원에 따라 다양해지는 가운데, 동거가족이 있는 노인의 인지적, 정서적, 사회적 자극을 줌은 물론이고 스트레스를 낮추는 역할을 해주며, 독거노인에 비해 동거인과 거주하는 노인이 정서적 지지의 영향 등으로 인지기능이 일정하게 유지되는 것으로 보고되었다&#x0005b;<xref ref-type="bibr" rid="b14-kjhp-2023-23-3-105">14</xref>,<xref ref-type="bibr" rid="b15-kjhp-2023-23-3-105">15</xref>&#x0005d;. 나아가 최근 12편의 논문을 분석한 체계적 고찰 결과, 독거노인인 경우 동거가족이 있는 노인에 비해 치매 발생 위험률이 1.3배 높은 것으로 보고되었다&#x0005b;<xref ref-type="bibr" rid="b17-kjhp-2023-23-3-105">17</xref>&#x0005d;.</p>
<p>한편 모든 노인이 시간 경과에 따라 동일한 속도와 유형으로 인지기능 감소가 일어나지는 않고, 이질적인 형태로 나타난다는 연구 결과가 보고되었다&#x0005b;<xref ref-type="bibr" rid="b4-kjhp-2023-23-3-105">4</xref>,<xref ref-type="bibr" rid="b8-kjhp-2023-23-3-105">8</xref>,<xref ref-type="bibr" rid="b16-kjhp-2023-23-3-105">16</xref>&#x0005d;. 즉, 대부분의 독거 노인에서 시간 경과에 따라 인지기능 저하를 보이는 경우도 있으나 일부 독거노인의 경우 높은 수준의 인지기능을 거의 변화가 없이 계속 유지하는 양상을 나타낸다고 보고한 선행 연구들도 있었다&#x0005b;<xref ref-type="bibr" rid="b10-kjhp-2023-23-3-105">10</xref>,<xref ref-type="bibr" rid="b11-kjhp-2023-23-3-105">11</xref>&#x0005d;. 대만, 미국과 호주, 필리핀 등에서 지역사회 거주 노인들을 대상으로 수행된 연구들의 경우 시간 경과에 따른 노인들의 인지기능 변화 양상이 3-7개로 다양하게 나타났다&#x0005b;<xref ref-type="bibr" rid="b4-kjhp-2023-23-3-105">4</xref>,<xref ref-type="bibr" rid="b10-kjhp-2023-23-3-105">10</xref>&#x0005d;. 이와 같이 노인들의 인지기능 변화 양상이 시간 경과에 따라 정형화된 또는 동질한 형태로 나타나지 않으므로, 노인 대상 인지기능 저하의 영향요인을 파악할 때, 시간 경과에 따른 집단 내 인지기능 변화 양상 또는 패턴을 고려하여 각 인지기능 변화 패턴별 영향요인을 고려할 필요가 있다&#x0005b;<xref ref-type="bibr" rid="b9-kjhp-2023-23-3-105">9</xref>,<xref ref-type="bibr" rid="b16-kjhp-2023-23-3-105">16</xref>&#x0005d;. 기존에 수행된 노인의 인지기능 관련 종단적 연구들은 심혈관 질환&#x0005b;<xref ref-type="bibr" rid="b18-kjhp-2023-23-3-105">18</xref>&#x0005d;이나 고혈압&#x0005b;<xref ref-type="bibr" rid="b19-kjhp-2023-23-3-105">19</xref>&#x0005d; 등 특정 만성 질환을 앓고 있는 노인을 중심으로 소수 이루어졌다. 한편 독거 인과 동거가족이 있는 노인 대상의 횡단적 조사 연구에서, 두 집단 간 인지기능에 통계적으로 유의한 차이가 없다고 보고한 연구 결과&#x0005b;<xref ref-type="bibr" rid="b12-kjhp-2023-23-3-105">12</xref>&#x0005d;와 동거형태에 따라 시간 경과에 따른 노인의 인지기능 변화에 유의미한 차이가 있다는 연구 결과들&#x0005b;<xref ref-type="bibr" rid="b11-kjhp-2023-23-3-105">11</xref>,<xref ref-type="bibr" rid="b15-kjhp-2023-23-3-105">15</xref>&#x0005d; 등 선행 연구들 간 일치하지 않는 연구 결과들이 보고되고 있다.</p>
<p>이에 본 연구에서는 지역사회 거주 노인의 동거형태에 따른 인지기능 변화궤적을 종단적으로 파악하고, 그 영향요인을 확인하므로써, 향후 독거노인 또는 동거가족이 있는 노인의 인지기능 변화 유형을 고려한 맞춤형 인지기능 향상 또는 치매 예방 프로그램 개발 시 기초 자료로 활용될 수 있을 것이다.</p>

<sec>
<title>2. 연구 목적</title>
<p>본 연구의 목적은 지역사회 노인의 동거형태에 따라 인지 기능 변화궤적의 이질적인 잠재계층 유형을 확인하고, 독거 노인과 동거가족이 있는 노인의 인지기능 변화 유형별 영향 요인을 파악하고자 시도되었다.</p>

<sec>
<title>방 법</title>
<sec>
<title>1. 연구 설계</title>
<p>본 연구는 고령화연구패널조사(Korean Longitudinal Study of Ageing, KLoSA)를 활용하여 지역사회 노인의 동거형태에 따른 인지기능 변화궤적의 잠재계층 유형을 확인하고, 인지기능 변화 유형별 영향요인을 파악하기 위한 종단적 조사 연구이다.</p>

<sec>
<title>2. 연구 대상</title>
<p>본 연구에 활용한 고령화연구패널조사는 고령자의 가족, 건강, 직업, 소득 등 전반적인 현황을 확인하기 위해 제주도를 제외한 45세 이상 성인을 대상으로 2006년부터 매 2년 주기로 시행되고 있다&#x0005b;<xref ref-type="bibr" rid="b20-kjhp-2023-23-3-105">20</xref>&#x0005d;. 본 연구는 국내 고령자 인구 비율이 15% 이상으로 고령사회가 된 2020년 고령 인구 추이 변화를 고려하여&#x0005b;<xref ref-type="bibr" rid="b1-kjhp-2023-23-3-105">1</xref>&#x0005d;, 2012년부터 2020년의 8년간 자료를 활용하여 대상자를 추출하였다. 본 연구의 대상자는 2012년 고령화연구패널조사에 참여한 7,486명 중 65세 이상 노인이면서 인지기능 검사 항목에 최소 3회 이상 참여한 3,244명을 대상으로 하였다. 이 중 가구 유형이 조사 시점과 최종 시점이 변경된 171명과 주요변수인 우울 항목에 미응답한 24명을 제외하고 최종 3,049명의 자료를 연구 분석에 이용하였다(<xref rid="f1-kjhp-2023-23-3-105" ref-type="fig">Figure 1</xref>).</p>
<sec>
<title>1) 인구사회학적 특성</title>
<p>인구사회학적 특성은 2012년 자료를 기준으로 성별, 연령, 교육 수준, 직업, 거주지역, 가구 월소득 수준을 포함하였다. 연령은 만 65세 이상을 기준으로 65-69세, 70-79세, 80세 이상으로 구분하였고, 교육 수준은 졸업을 기준으로 초졸 이하, 중졸, 고졸 이상으로 분류하였다. 직업은 조사 시점을 기준으로 유무를 구분하였고 거주지역은 주소지를 기준으로 동 단위는 도시, 읍&#x000b7;면 단위는 시골로 구분하였고, 가구 월소득은 전체 가구원의 월소득 총액을 기준으로 오분위로 분류하였다. 동거형태는 8년간의 가구 유형을 기준으로 단독 가구는 독거로, 부부 가구와 자녀동거 가구는 동거가족이 있는 자로 구분하였다.</p>
</sec>
<sec>
<title>2) 건강 관련 특성</title>
<p>건강 관련 특성은 초기 시점의 자료를 기준으로 현재 흡연 및 음주 상태, 체질량지수, 규칙적인 신체운동, 복합 만성 질환, 기본적 일상생활 수행 능력(activity of daily living), 도구적 일상생활 수행 능력(instrumental activity of daily living)을 포함하였다. 현재 흡연 및 음주 상태는 최근 1년 이내를 기준으로 유무를 구분하였고 체질량지수(kg/m<sup>2</sup>)는 2020년 대한비만학회 기준에 따라 18.5 미만은 &#x02018;저체중&#x02019;, 18.5-22.9는 &#x02018;정상&#x02019;, 23.0-24.9는 &#x02018;비만 전 단계&#x02019;, 25.0 이상은 &#x02018;비만&#x02019;으로 분류하였다&#x0005b;<xref ref-type="bibr" rid="b21-kjhp-2023-23-3-105">21</xref>&#x0005d;. 규칙적인 신체운동은 주 1회 이상 운동 여부를 기준으로 나누었고 복합 만성 질환은 13개 항목으로 분류된 만성 질환(고혈압, 당뇨, 암, 폐 질환, 간 질환, 심장 질환, 뇌혈관 질환, 정신과적 문제, 관절염 및 류마티스 질환, 전립선 질환, 소화기계 질환, 디스크, 치매) 중 2개 이상 의사로부터 진단받은 경험을 기준으로&#x0005b;<xref ref-type="bibr" rid="b22-kjhp-2023-23-3-105">22</xref>&#x0005d; 유무를 나누었고 기본적 일상생활 수행 능력과 도구적 일상생활 수행 능력은 도움 유무로 구분하였다.</p>
</sec>
<sec>
<title>3) 우울</title>
<p>우울은 한국판 단축형 역학연구 우울척도(center for epidemiological studies-depression scale-10, CES-D-10)를 활용한 초기 시점 자료의 점수로 측정되었다&#x0005b;<xref ref-type="bibr" rid="b23-kjhp-2023-23-3-105">23</xref>&#x0005d;. CES-D-10 도구는 최근 1일간의 느낌과 행동 정도에 관한 10가지 문항에서 &#x02018;잠깐 그런 생각이 들었거나, 그런 생각이 들지 않았음&#x02019;은 0점, &#x02018;가끔&#x000b7;자주&#x000b7;항상 그런 생각이 들었음&#x02019;은 1점으로 점수화되어 최대 점수 10점 중 4점 미만은 우울 없음, 4점 이상은 우울로 분류하였다&#x0005b;<xref ref-type="bibr" rid="b24-kjhp-2023-23-3-105">24</xref>&#x0005d;.</p>
</sec>
<sec>
<title>4) 인지기능</title>
<p>본 연구에서 인지기능을 측정하기 위해 한국판 간이정신검사(Korean mini mental state examination, K-MMSE)를 이용하였다&#x0005b;<xref ref-type="bibr" rid="b25-kjhp-2023-23-3-105">25</xref>&#x0005d;. 이 도구는 시간과 장소에 대한 지남력(10점), 기억등록(3점), 기억회상(3점), 주의집중 및 계산 능력(5점), 언어 능력(8점), 시공간 구성력(1점)의 11문항으로 구성되어 있으며, 총점은 30점 만점이다. K-MMSE는 점수가 높을수록 인지기능이 높음을 의미하며 24점 이상은 정상, 18-23점은 경도의 인지기능 장애, 17점 이하는 중증의 인지기능 장애로 구분하였다. 개발 당시 도구의 신뢰도는 Cronbach&#x02019;s &#x003b1;&#x0003d;0.86이었고, 본 연구에서의 신뢰도는 Cronbach&#x02019;s &#x003b1;&#x0003d;0.84였다.</p>

</sec>
<sec>
<title>4. 자료 분석</title>
<p>본 연구의 자료분석은 IBM SPSS version 26.0 (IBM Corp, Armonk, NY, USA)과 Mplus 8.7 프로그램(Muth&#x000e9;n &amp; Muth&#x000e9;n, Los Angeles, CA, USA)을 활용하였다. 대상자의 인구사회학적 및 건강 관련 특성, 우울, 인지기능은 빈도분석과 기술 통계를 이용하였다. 2012년부터 2020년까지 대상자의 인지 기능 변화궤적과 적합한 잠재계층 수를 결정하기 위해 잠재계층성장모형(latent class growth modeling)으로 분석하였다. 잠재계층성장모형은 시간의 흐름에 따른 개인의 변화량에 초점을 두고 한 집단 내에서 다양한 궤적을 보여주는데 유용한 방법으로 대상자의 인지기능 변화궤적이 상이한 계층을 확인하고 가장 적합한 잠재계층의 수는 모형 적합도를 의미하는 Akaike information criterion (AIC)와 Bayesian information criterion (BIC) 및 lo-mendell-rubin adjusted likelihood ratio test (LMR-LRT)를 기준으로 결정하였다&#x0005b;<xref ref-type="bibr" rid="b26-kjhp-2023-23-3-105">26</xref>&#x0005d;. 인지기능 변화 유형에 따른 대상자 특성 차이는 chi-squared test와 one-way analysis of variance로 실시하였고, 인지기능 변화 유형별 영향요인은 다항 로지스틱 회귀분석(multinomial logistic regression)을 실시하였다.</p>
</sec>
<sec>
<title>5. 윤리적 고려</title>
<p>본 연구에서 이용한 고령화연구패널조사(KLoSA) 자료는 한국고용정보원 홈페이지에서 연구자가 가입 후 자료를 획득하였고 공개된 자료에는 통계법 33조에 따라 개인을 식별할 수 있는 정보가 포함되지 않은 상태로 제공되어 있다. 본 연구는 연구자가 속한 기관의 연구윤리심의위원회에서 심의면제를 받은 후 수행하였다(IRB No. 1041078-20230411-HR-100).</p>

</sec>
<sec>
<title>결 과</title>
<sec>
<title>1. 대상자의 동거형태에 따른 인구사회학적 및 건강 관련 특성 비교</title>
<p>본 연구의 전체 대상자 3,049명 중 동거형태는 독거노인이 502명(16.5%), 동거가족이 있는 노인이 2,547명(83.5%)으로 나타났다(<xref rid="t1-kjhp-2023-23-3-105" ref-type="table">Table 1</xref>). 가구 월소득의 경우 독거노인은 하위수준인 1분위가 268명(53.4%)으로 가장 많은 반면, 동거 가족이 있는 노인의 경우 4분위가 635명(24.9)으로 가장 많았다. 도구적 일상수행 능력이 의존적인 경우는 독거노인이 31명(6.2%)으로 동거가족이 있는 노인 323명(12.7%)보다 두 배 이상 낮았고, 우울을 앓고 있는 경우는 독거노인이 100명(19.9%)으로 동거가족이 있는 노인 365명(14.3%)보다 더 많았다. 정상 수준의 인지기능 수준을 가진 독거노인은 283명(57.3%)으로 동거가족이 있는 노인 1,713명(68.7%)에 비해 더 낮았으며 이 외 인구사회학적 및 건강 관련 요인은 비슷한 비율을 차지하였다.</p>
</sec>
<sec>
<title>2. 동거형태별 인지기능 변화 유형</title>
<p>지역사회 노인의 동거형태에 따른 각각의 인지기능 변화 유형에 대한 가장 적합한 모형을 결정하기 위하여 궤적 수에 따른 AIC와 BIC 및 LMR-LRT 값을 확인하였다. 본 연구에서는 AIC와 BIC 값이 가장 작으면서 LMR-LRT 값이 0.05 미만으로 통계적으로 유의한 궤적을 모두 고려하여 독거노인과 동거가족이 있는 노인 각각 5개와 4개의 잠재계층으로 구분되는 모형을 최종 채택하였다(<xref ref-type="supplementary-material" rid="SD1-kjhp-2023-23-3-105">Supplementary Tables 1</xref>, <xref ref-type="supplementary-material" rid="SD2-kjhp-2023-23-3-105">2</xref>). 독거노인과 동거가족이 있는 노인의 인지기능 수준 초기 값은 각각 평균 23.45점, 평균 24.77점이었고, 기울기는 각각 평균 -0.79점, 평균 -0.67점으로 통계적으로 모두 유의하였다.</p>
<p>먼저 독거노인에서 최종 도출된 5개의 인지기능 변화 유형은 인지기능 초기 값과 시간 경과에 따른 변화 양상을 기준으로 class 1은 &#x02018;높음-유지형(high-stable)&#x02019;, class 2는 &#x02018;높음-저하형(high-declining)&#x02019;, class 3은 &#x02018;보통-급감형(moderate-steep declining)&#x02019;, class 4는 &#x02018;낮음-유지형(low-stable)&#x02019;, class 5 &#x02018;낮음-저하형(low-declining)&#x02019; 집단으로 분류되었고 class 1에 속하는 독거노인은 179명(35.6%)으로 가장 많았다(<xref rid="f2-kjhp-2023-23-3-105" ref-type="fig">Figure 2</xref>). 다음으로 동거가족이 있는 노인의 인지기능 변화 유형도 독거노인과 마찬가지로 class 1은 &#x02018;높음-유지형(high-stable)&#x02019;으로, 1,183명(46.4%)이 이 집단에 속하였다. Class 2, class 3, class 4의 경우 인지기능 초기 값이 각각 높은 수준, 중간 수준, 낮은 수준으로 시작하여 모두 시간 경과에 따라 점차 감소하는 양상을 보여 &#x02018;높음-저하형(high-declining)&#x02019;, &#x02018;보통-저하형(moderate-declining)&#x02019;, &#x02018;낮음-저하형(low-declining)&#x02019;으로 명명하고 분류되었다. 독거노인과 동거가족이 있는 노인 각각의 인지기능 변화 유형별로 인구사회학적 특성, 건강 관련 특성 및 우울의 차이를 살펴본 결과, 독거노인의 경우 성별(<italic>P</italic>&lt;0.001), 연령(<italic>P</italic>&lt;0.001), 교육 수준(<italic>P</italic>&lt;0.001), 직업(<italic>P</italic>&#x0003d;0.025), 거주지역(<italic>P</italic>&lt;0.001), 가구월소득(<italic>P</italic>&lt;0.001), 현재 음주(<italic>P</italic>&#x0003d;0.002), 규칙적인 신체운동(<italic>P</italic>&lt;0.001), 기본적 일상생활 수행 능력(<italic>P</italic>&#x0003d;0.001), 도구적 일상생활 수행 능력(<italic>P</italic>&lt;0.001), 우울(<italic>P</italic>&lt;0.001)에서 인지기능 변화 유형별에 따라 통계적으로 유의한 차이를 보였으나 현재 흡연, 체질량지수, 복합 만성 질환은 통계적으로 유의한 차이를 보이지 않았다. 한편, 동거가족이 있는 노인의 인지기능 변화 유형별로 모든 변인이 통계적으로 유의한 차이를 보이는 것으로 나타났다(<xref rid="t2-kjhp-2023-23-3-105" ref-type="table">Table 2</xref>).</p>
</sec>
<sec>
<title>3. 독거노인과 동거가족이 있는 노인의 인지기능 변화 유형별 영향요인</title>
<p>독거노인과 동거가족이 있는 노인의 인지기능 변화 유형별 영향요인을 파악하기 위해 단변량 분석에서 통계적으로 유의한 차이를 나타낸 변수들을 투입하여 다항 로지스틱 분석을 실시하였다. 독거노인에서 준거집단인 &#x02018;높음-유지형/저하형&#x02019;에 비해 &#x02018;보통-급감형&#x02019;에 속할 확률이 높은 경우는 80세 이상(odds ratio &#x0005b;OR&#x0005d;, 6.93; <italic>P</italic>&lt;0.001), 1분위 가구 월소득(OR, 4.19; <italic>P</italic>&#x0003d;0.021), 의존적인 도구적 일상생활 수행 능력(OR, 10.21; <italic>P</italic>&#x0003d;0.008)였다. 또한 독거노인의 준거집단에 비해 &#x02018;낮음-유지형&#x02019;에 속할 확률이 높은 경우는 80세 이상(OR, 2.54; <italic>P</italic>&#x0003d;0.005), 시골 거주(OR, 2.31; <italic>P</italic>&#x0003d;0.004), 1분위(OR, 2.91; <italic>P</italic>&#x0003d;0.026)와 2분위 가구 월소득(OR, 2.81; <italic>P</italic>&#x0003d;0.009), 우울(OR, 2.41; <italic>P</italic>&#x0003d;0.005)이었고 준거집단에 비해 &#x02018;낮음-저하형&#x02019;에 속할 확률이 높은 경우는 80세 이상(OR, 14.81; <italic>P</italic>&lt;0.001), 의존적인 도구적 일상생활 수행 능력(OR, 11.71; <italic>P</italic>&#x0003d;0.005), 우울(OR, 5.46; <italic>P</italic>&lt;0.001)이었다(<xref rid="t3-kjhp-2023-23-3-105" ref-type="table">Table 3</xref>).</p>
<p>동거가족이 있는 노인에서 준거집단인 &#x02018;높음-유지형/저하형&#x02019;에 비해 &#x02018;보통-저하형&#x02019;에 속할 확률이 높은 경우는 여성(OR, 1.84; <italic>P</italic>&lt;0.001), 80세 이상(OR, 5.33; <italic>P</italic>&lt;0.001), 초등학교 이하의 교육 수준(OR, 2.85; <italic>P</italic>&lt;0.001), 무직(OR, 2.09; P&#x02264;0.001), 시골 거주(OR, 1.78; P&#x02264;0.001), 25.0 kg/m<sup>2</sup> 이상의 체질량지수(OR, 1.61; <italic>P</italic>&#x0003d;0.004), 신체활동을 하지 않는 경우(OR, 1.46; <italic>P</italic>&#x0003d;0.009), 의존적인 기본적 일상생활 수행 능력(OR, 3.06; <italic>P</italic>&#x0003d;0.002), 의존적인 도구적 일상생활 수행 능력(OR, 1.82; <italic>P</italic>&#x0003d;0.002), 우울(OR, 3.08; <italic>P</italic>&lt;0.001)이었고 준거집단에 비해 &#x02018;낮음-저하형&#x02019;에 속할 확률이 높은 경우는 여성(OR, 1.86; <italic>P</italic>&#x0003d;0.036), 80세 이상(OR, 22.48; <italic>P</italic>&lt;0.001), 초등학교 이하의 교육 수준(OR, 3.47; <italic>P</italic>&#x0003d;0.003), 25.0 kg/m<sup>2</sup> 이상의 체질량지수(OR, 1.85; <italic>P</italic>&#x0003d;0.029), 신체활동을 하지 않는 경우(OR, 2.79; <italic>P</italic>&lt;0.001), 의존적인 기본적 일상생활 수행 능력(OR, 4.96; <italic>P</italic>&lt;0.001), 의존적인 도구적 일상생활 수행 능력(OR, 2.67; <italic>P</italic>&lt;0.001), 우울(OR, 4.87; <italic>P</italic>&lt;0.001)이었다(<xref rid="t4-kjhp-2023-23-3-105" ref-type="table">Table 4</xref>).</p>

</sec>
<sec>
<title>고 찰</title>
<p>노년기의 동거형태는 사회적 관계의 핵심 역할을 담당하며 인지기능 유지에 중요한 요인으로 최근 주목받고 있다&#x0005b;<xref ref-type="bibr" rid="b14-kjhp-2023-23-3-105">14</xref>&#x0005d;. 본 연구는 2012년부터 2020년까지 최근 8년간의 고령화연구패널 자료를 활용하여 지역사회 거주 노인의 동거형태에 따른 인지기능 변화 유형을 잠재계층 성장모형으로 확인하고, 독거노인과 동거가족이 있는 노인의 각각의 인지기능 변화 유형별 영향요인을 확인하고자 하였다. 본 연구 결과, 독거노인과 동거가족이 있는 노인의 인지기능 수준이 시간 경과에 따라 궤적의 기울기 차이를 보이며 하위유형이 서로 상이하게 이질적으로 나타났다.</p>
<p>먼저 본 연구에서는 지역사회 거주 노인의 인지기능 변화 유형은 초기 시점의 인지기능 수준과 변화궤적의 기울기 정도에 따라 독거노인은 높음-유지형, 높음-저하형, 보통-급감형, 낮음-유지형, 낮음-저하형으로, 동거가족이 있는 노인은 높음-유지형, 높음-저하형, 보통-저하형, 낮음-저하형으로 각각 도출되었다. 기존의 노인 인지기능 변화궤적에 관한 연구들에서 제시된 잠재계층 유형들을 살펴보면&#x0005b;<xref ref-type="bibr" rid="b27-kjhp-2023-23-3-105">27</xref>,<xref ref-type="bibr" rid="b28-kjhp-2023-23-3-105">28</xref>&#x0005d;. 시간 경과에 따라 인지기능 수준이 변화가 없는 유지형, 서서히 감소하는 저하형, 급격히 감소하는 급감형이 공통적으로 언급되고 있다. 본 연구 결과에서도 독거노인의 경우 인지기능 수준이 시간 경과에 따라 유지형, 저하형, 급감형 모두 나타났으나 동거가족이 있는 노인은 유지형과 저하형만 나타나 상이하였다. 이렇듯 동거형태에 따른 인지기능의 변화 양상이 상이하므로 노년기 인지기능 저하를 예방하기 위해서는 동거형태를 고려하여 차별적으로 접근하고 관심을 기울일 필요가 있겠다.</p>
<p>독거노인과 동거가족이 있는 노인의 각각의 인지기능 변화 유형별 영향요인들을 살펴보면, 독거노인의 경우 연령, 가구 월소득, 도구적 일상생활 수행 능력, 우울이 인지기능 변화궤적을 예측하는 요인이었고, 동거가족이 있는 경우 성별, 연령, 교육 수준, 체질량지수, 규칙적인 신체운동, 일상생활 수행 능력, 도구적 일상생활 수행 능력, 우울, 직업, 거주지역이 인지기능 변화궤적을 예측하는 요인이었다. 즉, 독거노인과 동거가족이 있는 노인 모두에서 연령, 도구적 일상생활 수행 능력, 우울은 잠재계층을 구분하는 공통된 영향요인이었다. 높음-유지형/저하형을 기준으로 80세 이상의 고령일 경우, 도구적 일상생활 수행 능력이 의존적일 경우, 우울이 있는 경우 다른 변화 유형에 속할 가능성이 높았고, 독거노인의 경우에만 도구적 일상생활 수행 능력은 낮음-유지형, 우울은 보통-급감형에서는 유의한 영향을 미치지 않았다. 이러한 결과는 86세 이상의 초고령자에서 인 기능이 빠르게 감소하며&#x0005b;<xref ref-type="bibr" rid="b9-kjhp-2023-23-3-105">9</xref>,<xref ref-type="bibr" rid="b11-kjhp-2023-23-3-105">11</xref>&#x0005d; 인지장애가 있는 노인에서 도구적 일상생활 능력 제한이 흔하다는&#x0005b;<xref ref-type="bibr" rid="b29-kjhp-2023-23-3-105">29</xref>&#x0005d; 선행 연구 결과들과 비슷한 맥락을 보였다. 또한 우울을 경험한 노인이 건강한 노인에 비해 인지기능 장애가 심하고 더 가속화한다는 결과와 맥락을 같이한다&#x0005b;<xref ref-type="bibr" rid="b10-kjhp-2023-23-3-105">10</xref>,<xref ref-type="bibr" rid="b29-kjhp-2023-23-3-105">29</xref>&#x0005d;. 따라서 보건의료인들은 도구적 일상생활 수행 능력과 우울이 인지기능 저하를 예측할 수 있는 중요한 요인임을 인식하고 도구적 일상생활 수행 능력 제한 정도와 변화를 감지하고 우울을 조기 선별하며 관리하는 노력이 요구된다.</p>
<p>한편, 독거노인의 경우 가구 월소득이 하위수준인 1분위일 때 높음-유지형/저하형에 비해 다른 변화 유형에 속할 가능성이 높았다. 낮은 소득 수준은 의료 혜택 제한과 사회 지원 부족으로 인해 인지기능을 감소시키고&#x0005b;<xref ref-type="bibr" rid="b6-kjhp-2023-23-3-105">6</xref>,<xref ref-type="bibr" rid="b17-kjhp-2023-23-3-105">17</xref>&#x0005d; 부적절한 자가 관리와 자기방임으로 이어질 수 있다는 결과와 일맥상통 한다&#x0005b;<xref ref-type="bibr" rid="b30-kjhp-2023-23-3-105">30</xref>&#x0005d;. 이에 저소득층 독거노인을 대상으로 다양한 사회적 지원 및 자가 관리 능력을 높일 수 있는 맞춤형 개입은 인지기능 저하를 예방하기 위한 효과적인 전략이 될 수 있겠다. 또한 동거가족이 있는 노인에서 여성인 경우, 초등학교 이하의 교육 수준인 경우, 체질량지수가 25.0 kg/m<sup>2</sup> 이상인 경우, 규칙적인 신체활동을 하지 않는 경우, 기본적 일상생활 수행 능력이 의존적인 경우 높음-유지형/저하형에 비해 보통-저하형, 낮음-저하형에 속할 확률이 높은 것으로 나타났다. 특히 동거가족이 있는 노인의 경우에서만 비만이면서 규칙적인 운동을 하지 않고 기본적 일상생활 수행 능력이 의존적일 경우 인지기능의 저하 위험성이 높아지는 것으로 나타났는데 이는 신체활동 부족과 제한된 기본적 일상생활 생활 수행 능력이 의존적일수록 인지기능 장애 위험성이 높았던 선행 연구 결과와 유사하였다&#x0005b;<xref ref-type="bibr" rid="b10-kjhp-2023-23-3-105">10</xref>,<xref ref-type="bibr" rid="b27-kjhp-2023-23-3-105">27</xref>&#x0005d;. 따라서 보건의료인들은 동거가족이 있는 노인의 인지기능 저하를 예방하기 위해 규칙적인 신체활동을 하도록 돕고 기본적 일상생활 수행 능력의 독립성을 유지하도록 동거하고 있는 가족과 함께 교육하는 것이 무엇보다 중요하겠다.</p>
<p>본 연구는 몇 가지 제한점이 있었다. 첫째, 2012년 고령화 연구패널 자료는 가구 유형을 기준으로 동거형태를 구분하였기 때문에 상대적으로 동거가족이 있는 노인의 비율이 독거노인에 비해 많아 본 연구 결과를 일반화하는 데 주의를 기울여야 한다. 둘째, 최근 8년간의 고령화연구패널 자료를 활용함으로써 과일/야채 섭취나 심리적 웰빙과 같은 유의한 독립변수를 사용하는 데 제한되었다&#x0005b;<xref ref-type="bibr" rid="b27-kjhp-2023-23-3-105">27</xref>&#x0005d;. 마지막으로, 본 연구는 동거형태를 단독가구와 부부/자녀 동거와 같은 가족을 중심으로 이분화하여 분석하였으나 세대계층과 가족구성원 외의 친구나 친척 및 기타 지인도 포함하여 좀 더 세분화시켜 다양한 동거형태에 따라 인지기능 변화궤적 및 영향요인을 살펴볼 필요가 있겠다. 이러한 제한점에도 불구하고 본 연구는 최근 8년간의 대규모 패널조사 자료를 활용하여 지역사회 거주 노인의 인지기능 궤적을 동거형태별로 확인하였고, 독거노인과 동거가족이 있는 노인 각각에서 인구사회학적 및 건강 관련 요인을 포괄하여 인지기능 변화 유형에 영향을 미치는 요인에 대한 정보를 제공하였다는 점에서 의의가 있다.</p>
